TO AUTHORIZE THE MAYOR TO ENTER INTO CONTRACT WITH MOUNT CARMEL CORPORATE HEALTH & WELLNESS TO PROVIDE INITIAL ASSESSMENT AND COACHING FOR ALL EMPLOYEES AND SPOUSES ELIGIBLE FOR THE GAHANNA HEALTHY MERITS PLAN.

WHEREAS, the City is beginning a new health and wellness initiative for 2009 for eligible employees and spouses; and
WHEREAS, with the new initiative the City would like to have on-site coaching services by a clinical exercise physiologist to help those eligible to attain their health and wellness goals; and
WHEREAS, Mount Carmel Corporate Health & Wellness can provide such on-site coaching services, and the Director of Human Resources has recommended we proceed with them;
NOW, THEREFORE, BE IT ORDAINED BY THE COUNCIL OF THE CITY OF GAHANNA, STATE OF OHIO:
Section 1. That the Mayor is hereby authorized to enter into contract with MOUNT CARMEL CORPORATE HEALTH & WELLNESS, 6150 East Broad Street, Columbus, Ohio 43213, for initial assessment and coaching for employees and spouses for the Gahanna Healthy Merits program for 2009; contract attached as EXHIBIT A, and made a part herein.
Section 2. That funding in the amount of SEVEN THOUSAND THREE HUNDRED TWENTY DOLLARS ($7,320) will be set aside in the 2009 Appropriations to cover the herein costs.
Section 3. That this ordinance shall be in full force and effect after passage by this Council and 30 days after date of signature approval by the Mayor.
