WHEREAS, the City is continuing their health and wellness initiative for 2011 for eligible employees and spouses; and
 
WHEREAS, with this initiative the City would like to continue to have on-site coaching services by a clinical exercise physiologist and a dietitian to help those eligible to attain their health and wellness goals; and
 
WHEREAS, Mount Carmel Occupational Health can provide such on-site coaching services, and the Director of Human Resources has recommended we proceed with them;
 
NOW, THEREFORE, BE IT ORDAINED BY THE COUNCIL OF THE CITY OF GAHANNA, STATE OF OHIO:
 
Section 1.  That the Mayor is hereby authorized to enter into contract with MOUNT CARMEL OCCUPATIONAL HEALTH OCCUPATIONAL SERVICES, 6150 East Broad Street, Columbus, Ohio 43213, for initial assessment and coaching for employees and spouses for the Gahanna Healthy Merits program for 2011; contract attached as EXHIBIT A, and made a part herein.
 
Section 2.  That funding in the amount of TWO THOUSAND EIGHT HUNDRED EIGHTY DOLLARS ($2,880) will be set aside in the 2011 Appropriations to cover the herein costs.    
 
Section 3.  That this ordinance shall be in full force and effect after passage by this Council and 30 days after date of signature approval by the Mayor.
