WHEREAS, the City is continuing the health and wellness initiative for 2013 for eligible employees and spouses; and
 
WHEREAS, with this initiative the City would like to continue to have on-site coaching services by a clinical exercise physiologist and a dietitian to help those eligible to attain their health and wellness goals; and
 
WHEREAS, Mount Carmel Workplace Health can provide such on-site coaching services, and the Director of Human Resources has recommended we proceed with them; and
 
WHEREAS, it is imperative to have the new agreement in place as soon as possible so there is no lapse of services, and all for the preservation of the public peace, property, health, safety and welfare;  
 
NOW, THEREFORE, BE IT ORDAINED BY THE COUNCIL OF THE CITY OF GAHANNA, STATE OF OHIO:
 
Section 1.  That the Mayor is hereby authorized to enter into contract with MOUNT CARMEL WORKPLACE HEALTH SERVICES, 6150 East Broad Street, Columbus, Ohio 43213, for initial assessment and coaching for employees and spouses for the Gahanna Gateway to Health Program for 2013; contract attached as EXHIBIT A, and made a part herein.
 
Section 2.  That funding in the amount of ONE THOUSAND EIGHT HUNDRED FIFTEEN DOLLARS ($1,815) was set aside in the 2013 Appropriations to Account No. 101.125.5281, Human Resources Wellness Plan, to cover the herein costs.    
 
Section 3.  That, for the reasons set forth in the preamble herein above, this ordinance is declared emergency legislation and shall be in full force and effect immediately upon passage by this Council and on date of signature approval by the Mayor.
