WHEREAS, the City believes it would be beneficial to offer life insurance to those part time employees scheduled to work 20 hours per week on a year round basis; and

WHEREAS, there are currently 57 part-time employees who would qualify for coverage; and

WHEREAS, the Director of Human Resources is recommending that the City offer a $10,000 life and accidental death and dismemberment policy to the eligible part-time employees;

NOW, THEREFORE, BE IT ORDAINED BY THE COUNCIL OF THE CITY OF GAHANNA, STATE OF OHIO:

Section 1. That the Mayor is hereby authorized to enter into Agreement with STANDARD INSURANCE COMPANY, 900 Southwest Fifth Avenue, Portland Oregon, 97204-1282 for life insurance coverage for eligible part-time employees; Agreement attached as EXHIBIT A.

Section 2. That funding in the amount of EIGHTY-EIGHT DOLLARS ($88) is hereby appropriated from the unappropriated, unencumbered balance of the Street Fund to the proper Life Insurance Accounts.

Section 3. That funding in the amount of THIRTY-NINE DOLLARS ($39) is hereby appropraited from the unappropriated, unencumbered balance of the Stormwater Fund to the proper Life Insurance Accounts.

Section 4. That funding in the amount of SIXTY-EIGHT DOLLARS ($68) is hereby appropraited from the unappropriated, unencumbered balance of the Water Fund to the proper Life Insurance Accounts.

Section 5. That funding in the amount of SIXTY-EIGHT DOLLARS ($68) is hereby appropriated from the unappropriated, unencumbered balance of the Sewer Fund to the proper Life Insurance Accounts.

Section 6. That funding in the amount of TWO THOUSAND EIGHT HUNDRED THIRTEEN DOLLARS ($2,813) is hereby appropriated from the unapporporiated, unencumbered balance of the General Fu...
