TO AUTHORIZE THE MAYOR TO ENTER INTO CONTRACT WITH MOUNT CARMEL OCCUPATIONAL HEALTH OCCUPATIONAL SERVICES FOR CORPORATE HEALTH & WELLNESS TO PROVIDE INITIAL ASSESSMENT AND COACHING FOR ALL EMPLOYEES AND SPOUSES ELIGIBLE FOR THE GAHANNA HEALTHY MERITS PLAN.

WHEREAS, the City is continuing the health and wellness initiative for 2012 for eligible employees and spouses; and
WHEREAS, with this initiative the City would like to continue to have on-site coaching services by a clinical exercise physiologist and a dietitian to help those eligible to attain their health and wellness goals; and
WHEREAS, Mount Carmel Occupational Health can provide such on-site coaching services, and the Director of Human Resources has recommended we proceed with them; and
WHEREAS, it is imperative to have the new agreement in place by February 1, 2012, and all for the preservation of the public peace, property, health, safety and welfare;
NOW, THEREFORE, BE IT ORDAINED BY THE COUNCIL OF THE CITY OF GAHANNA, STATE OF OHIO:
Section 1. That the Mayor is hereby authorized to enter into contract with MOUNT CARMEL OCCUPATIONAL HEALTH OCCUPATIONAL SERVICES, 6150 East Broad Street, Columbus, Ohio 43213, for initial assessment and coaching for employees and spouses for the Gahanna Healthy Merits program for 2012; contract attached as EXHIBIT A, and made a part herein.
Section 2. That funding in the amount of TWO THOUSAND EIGHT HUNDRED EIGHTY DOLLARS ($2,880) was set aside in the 2012 Appropriations to Account No. 101.125.5281, Human Resources Wellness Plan, to cover the herein costs.
Section 3. That, for the reasons set forth in the preamble herein above, this ordinance is declared emergency legislation and shall be in full force and effect immediately upon passage by this Council and on date of signature approval by the Mayor.
